We are committed to providing you with the best care available. As a condition of treatment by this office, financial arrangements must be made in advance. The practice depends upon reimbursement from patients for the cost incurred in their care. All emergency services, or any services performed without previous financial arrangements, must be paid for in cash at the time services are performed unless other arrangements are made. All lab case patients must make financial arrangements prior to appointment to reserve their chair time. Patients with dental insurance understand that all dental services are charged directly to the patient and that he or she is personally responsible for co-payment/payment of all dental services.
The office will help prepare the patient’s insurance forms or assist in making collections from insurance companies and will credit any collections to the patient’s account. However, this office cannot render services on the assumption that all charges will be paid by an insurance company. Some insurance plans may even require the patient to pay in full for services rendered at the time of service, and reimbursement will be sent directly to the patient.
